Instructions to Submit an Affidavit for Attorney General
Scroll down the following list and select the county in which you work or have a residence.
If you want your Witness Affidavit to become part of Thorne’s Complaint to the Attorney General, then download that Complaint under the “AG” button next to your county.
Repeat the following process for more than one Complaint or if you would to submit a complaint to the District Attorney, click here.
Print a hard copy of the entire eight page Thorne’s Complaint to the Attorney General.
Carefully read the entire Complaint, especially the eighth page that you were going to sign in front of the notary, but do not sign page 8 yet.
Notice in the top half of page 1 of the Complaint, there is contact information for the Attorney General, including his mailing and email address.
Take the hard copy of the entire eight page Complaint and your photo identification to a notary public.
Tell the notary public you want to sign an affidavit for them to notarize.
When the notary public asks you for it, present your photo identification to confirm your identity.
When the notary public tells you to do so, print your full name in the blank at the top of the Affidavit, then sign it, and then the notary public will sign and stamp it.
Then scan the entire eight page Complaint, including your signed and notarized page, into a PDF formatted digital file.
Then email the Complaint to the Attorney General at the email address you will find on page 1 of
Then mail a copy of the entire eight page Complaint to the Attorney General at the mailing address you will find on page 1 of Thorne’s Complaint to the Attorney General.
Set aside the original Complaint with your other important papers for your own records and for future reference, just in case you need to make additional copies, etc.
If you want to add your Witness Affidavit to Thorne’s Complaint to the District Attorney, then you do basically the same process, but you select Thorne’s Complaint to the District Attorney as follows.
If you want Thorne to know that you have emailed your Witness Affidavit to the Attorney General, then email a copy of it to Thorne at the following email address.
